Registration Procedures for 2011-2012


1.  Read the entire packet. Fill out the Registration Forms using the instructions below. Call an advisory board member if you have questions.
                                   
2.   Return the following completed forms together:
  • Registration Contract
  • Registration Form
  • Medical Release
  • Assumption of Risk Form
  • Alternative Learning Declaration
  • $50 Registration Fee (check or money order ONLY- no cash).
  •  Registrations will not be accepted unless all paperwork is complete and both parents have signed all forms.
